Advancing Access to Healthcare Through Transportation: The Health Connector Project Reaches Completion

Health Connector’s Nine-Stage Trip
In August 2026, the Health Connector project was completed, marking the successful closeout of a healthcare transportation initiative designed to make getting to and from medical care simpler and more reliable for Dallas County residents.
Each year, millions of Americans miss or delay medical care simply because they cannot get to it, a barrier that is often experienced most acutely by older adults, people with disabilities, and residents of rural communities, where limited transportation options, long travel distances, and sparse infrastructure can turn a routine appointment into a significant undertaking. The consequences reach far beyond the individual: more than 3 million people in the U.S. are unable to make their medical appointments as scheduled each year due to a lack of transportation access, and these no-shows cost hospitals an estimated $150 billion annually while contributing to poorer long-term health outcomes.
Health Connector was created to confront this challenge directly. By uniting patients and their caregivers, healthcare providers, and transportation services into a single coordinated journey, the initiative reimagined the full arc of a medical trip: from scheduling an appointment and traveling to the facility, to navigating once inside, to reaching a pharmacy and returning home, with the needs of companions and caregivers traveling alongside the patient considered at every step.
The project's origins trace back to a competitive federal grant. Flexlynqs President Santosh Mishra conceptualized and authored the ITS4US Deployment Program proposal while at IBI Group, which enabled HIRTA to secure approximately $5 million in funding as one of only five deployment sites selected nationally under the U.S. Department of Transportation program.
As the overall Systems Lead, Flexlynqs served as the integrating force behind the deployment, bringing together the project's many technologies and the partners responsible for each. Working hand in hand with healthcare, transportation, and technology collaborators, Flexlynqs led Health Connector from concept through implementation, ensuring every component worked together to deliver a seamless, end-to-end journey for travelers.
How It Works: The Technology Behind Health Connector
Health Connector brought together a suite of integrated technologies designed to make every stage of the medical trip more accessible and dependable.
At the traveler level, riders could book trips through multiple channels: a smartphone app, an interactive voice response (IVR) phone line, or a self-service kiosk installed at Dallas County Hospital, ensuring that no one was excluded based on their access to technology. Once a trip was booked, riders received real-time updates on vehicle status and estimated arrival times, reducing uncertainty and allowing them to plan their day with confidence.
On the ground, drivers operated with dedicated tablets that provided turn-by-turn navigation, just-in-time dispatch instructions, and pickup wayfinding tools, supporting consistent, on-time service delivery across Dallas County's rural network.
Inside healthcare facilities, a NaviLens wayfinding system used QR-code-based navigation accessible through any smartphone camera to guide riders from vehicle drop-off to their appointment destination, a tool designed specifically to support individuals with visual impairments, limited English proficiency, or limited familiarity with large medical facilities.
For healthcare partners, a dedicated monitoring platfom gave Dallas County Hospital coordinators and health navigators real-time visibility into patient appointments and transportation status in a single view. Coordinators could monitor upcoming appointments and trips, and book transportation directly on behalf of patients, reducing administrative burden and helping prevent missed appointments before they happen.
The system was designed from the outset with replicability in mind, ensuring that other communities facing similar healthcare transportation challenges could learn from and build on what Health Connector developed. Key technologies and integration frameworks developed through the project have been made openly available, lowering the barrier for other agencies looking to implement coordinated medical transportation solutions of their own.

Preliminary Individual-Level Outcomes Over Initial 18 Months of Service
What the Data Shows
Over 18 months of service, Health Connector delivered measurable results for the travelers it was built to serve. Riders confirmed that available trip options met their needs almost 100% of the time, and the service achieved near-perfect on-time performance, both exceeding the project's 90% target. Just as telling were the numbers that stayed low: near-zero rider complaints were reported and no-show rates remained minimal throughout the deployment, demonstrating that when people have access to dependable transportation, they show up for the care they need.
The Road Ahead
Central Iowa proved a meaningful setting for this work. Its blend of urban centers and rural communities mirrors the transportation realities of much of the country, making the lessons learned here applicable well beyond the region, and USDOT is now funding others to build on that foundation through the ITS4US Accelerator program. Health Connector stands as a powerful demonstration of what cross-sector collaboration can achieve: when transportation, healthcare, and technology are thoughtfully integrated, communities can dismantle long-standing barriers to care.
